Blackhawk Transport has a variety of resources for employees. You can always contact the Blackhawk Transport HR team directly during business hours. However, we know you need convenient access to your payroll and benefits information 24/7. With over 650 drivers on the road throughout the country, we use our electronic systems to facilitate enrollment in our benefit plans and to communicate critical employee information.

 

Paylocity

PAYLOCITY

The most important payroll and benefits information for our employees is PAYLOCITY. Paylocity is the Blackhawk Transport human resource and payroll information system. You will create an account in Paylocity to view your pay stubs, tax information, and to verify your personal data including your benefit enrollment/status. Paylocity also has an app feature that can be utilized for quickly referencing items; however, it cannot access your benefit enrollment. Please use the website to view your benefits. 


The first time logging into Paylocity, you will need to click on “Register New User” and follow the prompts. For more detailed instructions, please see the benefits guide provided during orientation or call Human Resources at 608-364-4040 for additional assistance. 


We recommend you bookmark this location and memorize your company ID, username, and password. If you forget your password, there is a “Forgot Password” feature on the website that sends an email within 30 minutes containing a link that allows you to reset your password. The email is sent to the address you have registered with the company.

During orientation, we will review your employee benefit options in detail. We know it can be overwhelming, so we offer multiple ways for you to get information about your benefits.

If you are enrolled in the Blackhawk Transport health insurance program, you can access your benefit information online at Anthem Blue Cross. Members can print ID cards, view claims, check on deductibles and copays, and manage prescriptions. You can also find providers and important health care information, as well as speak directly to a representative.

You can establish your account with Anthem once the first payroll deduction for your health benefits is deducted from your paycheck. 

 

Delta Dental & Vision

Blackhawk Transport dental and vision benefits are managed by Delta Dental and EyeMed Vision Care\Delta Vision. If you are enrolled in dental or vision insurance, you can access your benefits information and identification cards here: 

 

Employee Benefits Corporation

Flex Spending

Blackhawk Transport employees can take advantage of a pre-tax flexible spending account to pay for qualified medical, dental and vision expenses including deductibles, copays and other out-of-pocket expenses. You can access the employee portal and track your spending and available balance in real time.

 

Alliance Plan

 

401(k)

Blackhawk Transport offers a 401(k) retirement savings plan with matching contributions made by the company. Employees are eligible after six months of service, and we will notify you when you are eligible. To establish your individual 401(k) account, set your deferral contribution and determine your investment elections

 

Employee Resource FAQ's

You are able to cover any legal dependent(s).

Your spouse, the person who is legally married to the employee while the employee is covered under the plan. Common law marriages are not valid for the purpose of the plan. Our medical plan has a spousal carve-out policy.

Dependent children up to age 26,including the employees’ natural child, legally adopted child (including the period of probation when the child is placed with adopting parents), a stepchild for whom your spouse is required to insure,, a foster child and a child whom you must cover due to a Qualified Medical Child Support Order, subject to the conditions of the law. 

An unmarried child who is physically or mentally incapable of self support upon attaining age 26 may be covered under the plan, if they remain incapacitated and unmarried. The child will be considered a covered dependent if he or she was disabled prior to their 26th birthday. Evidence of dependent eligibility may be requested. 

When your legal spouse has the option of obtaining health coverage through their own employer, your spouse will only be allowed to enroll in the company plan for secondary coverage. This means they must obtain health coverage through their own employer first and use it as primary coverage.

Eligible employees are able to sign up for their elected benefits within 30 days from their hire date on the Paylocity website (not through the mobile app). 

If a person chooses to not elect any benefits, then they will be automatically enrolled in Group Life and Accidental Death & Dismemberment (AD&D) Insurance, Short Term Disability, and Long Term Disability. However, the employee would still be responsible for providing appropriate beneficiary information for company-paid Life Insurance, which can be updated at any time on the Paylocity website portal. Again, Group Life, AD&D, Short and Long Term Disability are paid by the company with no cost to the employee

Blackhawk Transport has a waiting period for benefit eligibility: the first of the month following 60 days of full time employment. 

For optional benefits, other than 401(k), if you do not enroll during your initial waiting period of employment, you will not be able to enroll until the next open enrollment period, unless you have a qualifying event.

 You are eligible to enroll at any time (this is not an automatic enrollment). This means if you have an active 401(k) elsewhere, you can roll over those funds prior to being eligible to start contributing.

You are eligible to contribute toward your 401(k) the first of the month following six months of employment.

You have a 30-day period to provide documentation for such an event within the special enrollment period. Please reach out to Human Resources to discuss.

We hold open enrollment during the fourth quarter every year. You will receive information from Human Resources leading up to the open-enrollment period and friendly reminders via company communications. These elections are what you are choosing for the new upcoming yearly benefit program.

We partner with a benefit resource center you can call toll free at 855-874-0829 8 a.m. to 5 p.m. EST & CST Monday-Friday.

  • Your regular work schedule is permanently reduced to less than 30 hours per week.
  • Your employment ends due to resignation, termination, or death.
  • When your coverage ends.
  • The last day of the month in which the dependent is no longer eligible (up to age 26 or legally allowed).
  • The last day of the month in which a dependent is deemed ineligible.
